
RETIN-A
Retin-A is a derivative of vitamin A which has been shown in clinical trials to improve wrinkled skin. As discussed previously, skin wrinkling and aging is primarily due to the effects of sun and environmental exposure. Loss of collagen and breakdown of elastic fibers are the biological components of the aging process. Long term use of Retin-A causes the deposition of new collagen and the reduction of wrinkling. In addition, Retin-A reverses some of the pre-cancerous changes seen in sun-damaged skin.
Retin-A is a prescription drug that comes in several strengths. I prefer to begin with a lower strength and gradually increase the dosage. The drug is applied as a cream. Redness and slight scaling are expected with proper use. It is best applied in small amounts on clean, dry skin at bed time, and washed off in the morning. Application is suggested every third night in the beginning, but may be used more often as tolerated. Constant use for at least nine months is required before much improvement is seen, and use may be continued for years.
It must be emphasized that Retin-A is not a substitute for chemical peeling, laser resurfacing, or face lift surgery. Retin-A has no effect on the sagging and bagging of skin such as seen in the neck and jowl region. Retin-A will be insufficient for treatment of deeper wrinkling such as seen around the lips and fore head. Only a chemical peel or laser resurfacing can significantly improve this type of wrinkling. Retin-A is primarily useful for fine wrinkling and as an overall "skin polisher". Whether or not Retin-A is suitable for your skin is best determined at consultation.
